CSTA Horizons ETFs Golf Tourney

The golf tournament will be taking place on Saturday, August 14, 2010 at Whistler Golf Course in Whistler, British Columbia.  Golf starts promptly at 1:00pm.  ONLY REGISTERED ATTENDEES CAN PLAY GOLF! Registrants will receive confirmation of their status as a participant.

Golf Tournament Fees: $225 (includes boxed lunch, power cart and all taxes)

Full Nike sets are available for both men and women.  Rental charges are $50 ( plus  applicable taxes). Please complete the following form if you require rentals. 

GOLF CLUB  RENTALS:  REQUESTS FOR GOLF CLUB RENTALS MUST BE MADE BY TUESDAY, AUGUST 10, 2010 BY 5PM EST.
